What is exercise referral?
An exercise referral scheme’s purpose is to support those who have one or more health conditions or risk factors that can be supported with exercise. Traditionally, these are hosted in local communities and leisure centres. However, a well structured scheme can be difficult to locate or attend. Optimal Fitness Health and Conditioning will be expanding current services to include a blended approach of home and virtual sessions.

We can appreciate why this may be a challange for some. Many services are fantastic and operate out of your local leisure centre, however if this is not for you and are interested, Optimal Fitness Health and Conditioning can provide exercise referal at home – face-to-face or virtually (or both!).

Referral Process
Self-referral: You are able to contact Optimal Fitness Health and Conditioning directly to discuss whether exercise referral is right for you. Currently, there is no funding or support for this service, but this may change in the future. If this occurs, it will be reported here. Please call to discuss your options as bespoke packages can be developed, supporting most needs.
Medical Referral: You can be referred by primary care or allied health professionals. The GP or health professional will work with you and help you decide what is best for you and your health condition. However, we are new, so please direct your medical professional to this website or ask them to call for a discussion to support you.

Consultation and Assessment: This will allow us to identify your personal goals and set realistic targets and start-points that will support you and your health.
Personalised Exercise: Based on the initial consultation and assessments, the programme will be designed with you and adjusted over a period of time to allow progression. This is a very individual process that considers your health and capability.
Ongoing Monitoring and Support: Alongside incremental adjustments, you can expect your programme to change as you improve. Assessments can be repeated so that you can see the progress.
Analysis and Feedback: You are central to this and have every right to expect objective feedback on the reasons for your programme design choices. Furthermore, if you are a medical referral, there is an option for Optimal Fitness Health and Conditioning to provide a report to your GP or medical practitioner.
Behaviour Change: Alongside your clinical support, recognising and adjusting behaviour is important for long-lasting positive health outcomes. Behaviour change principles employed will consider your current motivation, needs and goals.
